The State of Connecticut requires all children who attend licensed child care and nursery school programs see a physician on a yearly basis. Please use the State of Connecticut Health Assessment Form for ages birth to age 5 when you take your child to his/her annual physical and have your physician fill it out. Part I must be completed by the child’s parent or guardian and Part II must be completed by the doctor.
